Who is Holisticare Hospice?
Hospice is not a place, but an approach to end of life care that provides comfort and a dignified end-of-life experience to patients and their families. Hospice is provided where a patient lives, whether that is a personal residence; a long term care facility; an assisted living facility or a hospital. Often times, electing hospice is misperceived as giving up. However, choosing hospice is a decision that comes from a place of strength, courage and acceptance. When the choice is made for a patient to spend their remaining time in a comfortable environment, surrounded by loved ones, pursuing palliative pain relief and comfort, while making end of life preparations, there is a sense of peace, for the patient and their family.
The mission of Holisticare Hospice ensures that patients and their family have an end-of-life experience that provides both physical and spiritual comfort provided by a team of experienced professionals.
Our Commitment
The Holisticare Hospice Team embodies the belief that end-of-life care is a collaborative effort, between the Hospice Team; the patient; his/her caregivers and family members. The RN/Case Manager works closely with the rest of the Interdisciplinary Team, including a medical director; social worker; and chaplain, as well as facility professionals and attending physicians to ensure the development of a Care Plan that best serves the patient and his/her family.
